Our story

Heritage Health Services has been providing care for residents in Port Washington and Ozaukee County since 1966. Our staff provides skilled and compassionate therapy and rehabilitation, and we’ve developed a robust activities program to keep our residents engaged. Our goal is to create an environment that affords our residents the opportunity to be a partner in the care they receive, to develop trusting relationships with our caregivers, and to reach their highest level of independence.
